Opaque, lazy media reference on an inbound event. The bytes are NOT inline; IProvider.downloadMedia(ref) retrieves them. This single shape covers:

• Baileys (decrypt + CDN fetch + possible reupload),
• Cloud API / Telegram (resolve id -> short-lived URL -> fetch bytes),
• Discord (plain HTTPS GET).
